Community Support Case Management Services

Beyond Residential Care offers Section 17 case management services. Our case managers will provide advocacy, support and guidance to clients struggling with a wide range of mental health and psychosocial challenges. Case managers will assist clients with access to resources needed to increase capacity for independence and enhance general well-being.

Case managers can provide support in several ways including access to:

Basic needs such as housing, food, hygiene items and clothing.

Support Opportunities Include:

Coordination of Care with medical and behavioral providers

Linkage to vocational opportunities, educational support, and skills training

Linkage to additional community and financial resources

Linkage to Support groups, intensive outpatient programs, and/or higher levels of care

Linkage to financial and legal assistance

In order to qualify for Section 17 services, all clients must have:

Full MaineCare

A DSM 5 diagnosis, or

Have a diagnosis that contributes to the risk for homelessness, psychiatric hospitalization, residential treatment, or criminal justice involvement.
