Community Support Case Management Services
Beyond Residential Care offers Section 17 case management services. Our case managers will provide advocacy, support and guidance to clients struggling with a wide range of mental health and psychosocial challenges. Case managers will assist clients with access to resources needed to increase capacity for independence and enhance general well-being.
Case managers can provide support in several ways including access to:
Basic needs such as housing, food, hygiene items and clothing.
Support Opportunities Include:
● Coordination of Care with medical and behavioral providers
● Linkage to vocational opportunities, educational support, and skills training
● Linkage to additional community and financial resources
● Linkage to Support groups, intensive outpatient programs, and/or higher levels of care
● Linkage to financial and legal assistance
In order to qualify for Section 17 services, all clients must have:
● Full MaineCare
● A DSM 5 diagnosis, or
● Have a diagnosis that contributes to the risk for homelessness, psychiatric hospitalization, residential treatment, or criminal justice involvement.
